To schedule your first appointment, please register as a new user in our secure patient portal.

While the process takes a few minutes, completing it now ensures that your first appointment is focused, thorough, and valuable. The questions in our intake form are different from a standard doctor’s office—for good reason. They help us get to the root of your health goals from day one.

The portal also allows you to:

  • Securely message with Dr. Carriere

  • Schedule and manage appointments

  • Pay bills

  • Share documents (HIPAA-compliant)

📝 How to Register

  1. Visit: https://imcportal.md-hq.com/registration

  2. (optional) Download these registration instructions as a PDF for easy reference. Registration Instructions PDF

  3. Complete the form and click “Sign Me Up!”

🔧 After Registering, Please Do the Following:

  1. Add Your Pharmacy. Go to My Pharmacies → Add a Pharmacy → enter your preferred pharmacy details.

  2. Fill Out Forms & Questionnaires. Navigate to Forms & Questionnaires → complete all forms listed.

  3. Schedule Your First Appointment. Go to My Appointments → choose Dr. Edward Carriere as your provider → select “New Client Intake Consultation” → pick a date and time that works for you.
